If you are coming to your first counseling appointment, please review and complete the following forms, and bring them with you:

You only need to complete the authorization for release of information if you want me to be able to speak with someone else about your treatment.

 

Parents of clients under age 18 should complete the supplemental form for adolescent clients as well.  I look forward to meeting with you.
